If your ultrasound report describes a hypoechoic thyroid nodule or a “hypoechoic mass,” it simply means the nodule looked darker than the tissue around it. Echogenicity is one of five features that radiologists score with the ACR TI-RADS system, and how dark a nodule appears helps estimate its risk. What “hypoechoic” actually means

On a thyroid ultrasound, sound waves bounce back from tissue and create shades of gray. Normal thyroid tissue reflects a moderate amount, so it looks medium-gray. When a nodule reflects less sound than the surrounding thyroid, it appears darker, and radiologists call it hypoechoic. A nodule that is even darker than the strap muscles in the neck is called very hypoechoic.

  • Anechoic — pure black, usually a fluid-filled cyst. Very low risk.
  • Hyperechoic or isoechoic — brighter than or equal to normal thyroid. Lower risk.
  • Hypoechoic — darker than normal thyroid. Moderately concerning.
  • Very hypoechoic — darker than neck muscle. The most concerning echogenicity.

Why echogenicity matters for cancer risk

Darker nodules are scored higher because studies link low echogenicity to a greater chance of thyroid cancer. Many thyroid cancers, especially papillary thyroid carcinoma, tend to appear hypoechoic. That does not mean a hypoechoic nodule is cancer. Most are benign. It only means the feature earns points that add up with the other four categories.

In the ACR TI-RADS point system, echogenicity is scored on its own, then added to composition, shape, margin, and echogenic foci.

Hypoechoic does not equal a diagnosis

A single dark nodule is not enough to judge risk. A solid, hypoechoic nodule that is also taller-than-wide with an irregular margin scores much higher than a hypoechoic nodule that is otherwise smooth and oval. What happens next

Whether a hypoechoic nodule needs a biopsy depends on its total TI-RADS category and size, not on echogenicity alone.
